Is my physician trying to commit insurance fraud?

I went to my doctors yesterday and paid for the visit (my insurance policy only covers 3 visits each year so I didn’t want to waste it on this visit). All the doctor did was check my temperature and gave me a referral to get a blood test & an x-ray. She said she wanted to get the test results back before she gave me any medication.
I called her after my x-ray & blood test to ask her to fax the prescription to my pharmacy (after she gets a look at the results) so I could pick it up later. She said that she wants to see me tomorrow in order to give me my prescription and told me to either bring my insurance or for the visit.
Is this considered health care insurance fraud? If so, is there any way I can call her out on it? I thought follow-ups are usually free of charge.
